Company's can make a referral by clicking the link below. 

A Personal Fatigue Consultations are designed to identify the work and non-work related variations that affect individuals in the workforce, identify their current management strategies, and develop recommendations for additional strategies that could be implemented to proactively manage fatigue, health and wellbeing.

Referral Process

  1. Referral is triggered by either
    - worker logs fatigue events
    - worker seeks assistance to help improve sleep/health
    - worker flagged on order43/ or other medical assessment
  2. HS officer discusses above with worker, gains consent to refer worker to fatigue program
  3. HS officer navigates to referral link or fatigue tech website (wherever we are going to put the referral link)
    - HS officer enters their details
    - HS officer enters the worker they are referring details
  4. HS officer completes the referral online and receives a referral receipt
    - Worker is automatically sent an email with unique link to complete PFA directed to applicable resources
    - If worker meets sleep referral criteria this is automatically generated at completion of PFA and sent to their email
